If you are already a patient with us you can also register for Online Patient Access by completing the following form. Online Registration Application Form This form can also be collected at reception.
You will need to hand this from in at reception together with photo ID or alternatively you can complete and email to the following address
lscicb-gp.p81015.reception@nhs.net together with copy of photo ID.

Please be aware that some of the formatting of Patient Access with regards to medication has apparently recently changed, one of our patients has asked us to pass this information on to everyone, in the hope that if you are struggling to find this it may help you.
The opening page of Patient Access will open up with ‘ Hello (Name)’ in the top L H corner.
At the bottom of this page in the centre it will say ‘Patient services’ press on this and it will open a page with 6 symbols of various services.
4th symbol down is titled ‘ Order repeat medication ‘ .
Press this symbol and it will open a page with on the LH column headed ‘ Your orders ‘ and the RH column headed ‘ Your medication ‘.
You can now, as before, order your repeat medication.

If you're both patients at our surgery, you can apply to help someone manage their GP services or choose someone to help manage yours.
GP services are things like booking appointments, ordering repeat prescriptions and viewing medical records. You choose the services you want to get or give access to.
