Online 'Registration' With the Practice

complete the form online and submit and our administration team will then start the registration process, when this is complete, we will send you a welcome text to your mobile, if you do not have one then please call us to check your status, after a period of time.
Please note if you are registering with a NHS doctor for the first time as you have recently arrived in the country, AND you take prescribed medication, we will need to see proof of this medication, from your previous doctor and you may possibly need to see one of our doctors or our clinical pharmacist before we will issue any further medication.

Please note registrations can take up to 14 days to be processed once we have received all the registration documents. However we endeavour to process this as quickly as possible
When you visit the surgery for the first time you may be asked to sign the forms to confirm that the details are correct.
Note that by sending the form you will be transmitting information about yourself across the Internet and although every effort is made to keep this information secure, no guarantee can be offered in this respect.
PLEASE NOTE - If your previous surgery is outside of England and you are on regular medication, you will need to provide proof of this medication before we can issue you a new prescription, this may be done by emailing or bringing in a photo or original box of the medication with a recent date on. You can also provide us with a medical print out from your last surgery, please note that this must be in English. Thank you.
Online Patient Access
If you are already a patient with us you can also register for Online Patient Access by completing the following form Online Access Application Form. This form can also be collected at reception.
You can either send the form and proof of identity via email to: lscicb-gp.p81015.reception@nhs.net or
drop the form into reception along with ID, which we will take a copy of.
We will aim to process this within 10 working days or sooner if we can.
